Vaccine Clinics

Here you will find a list of Covid-19 Vaccination Clinics that we are coordinating for our Native Hawaiian and Pacific Islanders in Utah.

  • Vaccines are currently being distributed for adults 65 and older.

  • Beginning March 1st, individuals ages 16 and older that have a qualified medical condition will be eligible to receive the vaccine. Please note, 16 and 17 year olds are currently only eligible to receive the Pfizer vaccine.

CLINIC SCHEDULE

We are currently offering clinics on the following dates, please scroll down for more specific details and eligibility for each clinic as it may vary.

  • Saturday, February 27, 2021 First dose distribution for ages 65 and older

  • Saturday, March 6, 2021 First and Second dose distribution for those that qualify

  • Saturday, March 13, 2021 First and Second dose distribution for those that qualify

  • Saturday, March 27, 2021 First and Second dose distribution for those that qualify

After pre-registering, save time by completing and bringing the Registration/Consent Form with you to the clinic.

SATURDAY, FEBRUARY 27, 2021

LOCATION

Sauniatu Samoan Ward (Taylorsville Stake Center), 4950 S 1950 W, Taylorsville, UT 84118

ELIGIBILITY

First dose distribution for Native Hawaiian and Pacific Islanders:

  • Ages 65 and older -or-

Mana Academy, 2355 S Technology Drive, West Valley City, UT 84120

Second Doses: 10:00am-12:00pm (only for those individuals that received their first dose at MANA Academy on 2/6/21)

First Doses: 12:00pm-2:00pm

ELIGIBILITY

First dose distribution for Native Hawaiian and Pacific Islanders that meet one of the following criteria:

  • Age 65 and older -or-

  • Ages 16 and older that have a qualified medical condition. (See eligibility details below to see if you qualify). Note, ages 16 and 17 year olds that are eligible, must obtain a parent signature to receive the vaccine at this clinic, Pfizer vaccines will be distributed at this clinic.

Second dose distribution will only be given to individuals that received their first dose at MANA Academy on Saturday February, 6, 2021. Please remember to bring your vaccination card with you to the clinic.

First Doses: 12:00pm-2:00pm

ELIGIBILITY

First dose distribution for Native Hawaiian and Pacific Islanders that meet one of the following criteria:

  • Age 65 and older -or-

  • Ages 18 and older that have a qualified medical condition. (See eligibility details below to see if you qualify). Note, the vaccine will not be distributed to ages 16 and 17 years old for this clinic.

Second dose distribution will only be given to individuals that received their first dose at the Tongan West Stake on Saturday February, 13, 2021. Please remember to bring your vaccination card with you to the clinic.

ELIGIBILITY & REQUIREMENT DETAILS

Second Dose

Second doses will be distributed only to individuals that received a 1st dose at one of our clinics. You must receive your second dose from the same location that you received your first one as follows:

  • First dose received on 2/6/21 at Mana Academy, second dose will be given on 3/6/21 at Mana Academy.

  • First dose received on 2/13/21 at Tongan West Stake, second dose will given on 3/13/21 at Tongan West Stake.

  • First dose received on 2/27/21 Sauniatu Samoan Ward, second dose will be given on 3/27/21 at Sauniatu Samoan Ward.

First Dose

Native Hawaiian & Pacific Islanders that meet one of the following criteria are eligible to receive the vaccine:

  • 65 years and older -or-

  • 16 years and older that have one of the following medical conditions as outlined on the state's eligibility requirements:

    • Uncontrolled diabetes with an A1c of 9% or higher

    • Receiving dialysis for severe kidney disease

    • Body Mass Index (BMI) of 40 or higher (this is also called Class III or severe obesity)

    • Chronic heart disease (not hypertension) including chronic heart failure, ischaemic heart disease, and severe valve or congenital heart disease

    • Stroke and dementia (Alzheimer’s, vascular, or frontotemporal)

    • Stage 4 or stage 5 chronic kidney disease

    • Solid organ transplant recipient

    • Receiving immunosuppression therapy

    • Asplenia including splenectomy or a spleen dysfunction

    • Chronic liver disease including chronic hepatitis B or C, alcohol-related liver disease, primary biliary cirrhosis, or primary sclerosing cholangitis or hemochromatosis

    • Cancer diagnosed within the last 5 years that began in the blood, bone marrow, or cells in the immune system. This type of cancer is called hematologic cancer (such as leukemia, lymphoma, and multiple myeloma).

    • Cancer diagnosed within the last 1 year that didn’t begin in the blood or bone marrow. This type of cancer is called non-hematologic cancer (excluding basal and squamous cell cancer diagnoses).

    • Immunocompromised state (weakened immune system) from blood, bone marrow, or organ transplant; HIV; long-term use of corticosteroids; or other medicines that weaken the immune system

    • Neurologic conditions that impair respiratory function, including cerebral palsy, Down syndrome, epilepsy, motor neuron disease, multiple sclerosis, myasthenia gravis, Parkinson’s disease, progressive cerebellar disease, and quadriplegia or hemiplegia

    • Sickle cell disease

    • Severe chronic respiratory disease (other than asthma) including severe chronic obstructive pulmonary disease, fibrosing lung disease, bronchiectasis, or cystic fibrosis

Important Notes

Ages 16-17 years with underlying issues need an adult signature to receive the vaccine and can ONLY get the Pfizer vaccine.
